The Centivo Partnership Plan is designed to offer high-quality care at prices that don’t break the bank. By focusing on a relationship between you and your primary care doctor – which can help keep you healthier, improve your healthcare experience and reduce emergency room visits – your costs stay lower without sacrificing quality of care.Â
Who is this program for?
You should consider the Centivo Partnership Plan if any of the following apply:
- You live in CA, CO, CT, FL, NJ, NY, PA, Dallas, TX and WI
- You want to save money on your healthcare.
- You already see doctors in the Centivo Network or are open to switching doctors.
- You don’t have regular doctors you see today.
- You typically use urgent care for your medical needs.
- You want the convenience of a virtual primary care practice.
- You have dependents under 18 who need routine pediatric care. Pediatrician visits are free.
- You know you’re going to have a baby or need surgery. This plan offers low costs and a low out-of-pocket maximum, so you’ll pay less out of pocket for these major medical events.

There is also a penalty charge, equal to the copay amount listed, if you have a specialist office visit or have surgery without getting a referral from your designated primary care doctor. No referral needed for OB/GYN, behavioral health, urgent, emergency or chiropractic care, lab work, physical, occupational or speech therapy. |
